I have lost my PAN card. Do I need to apply for duplicate or new PAN card? What is the step?

Two things are required: PAN Number (or PAN Card).

PAN Number remains the same even if you loose your PAN Card. It is unique and only one number. An person cannot hold multiple PAN Number. This could lead to a penalty of Rs 10,000 and other legal actions.

You can request a new 'PAN Card' if you've lost yours. The form can be downloaded from the official website NSDL and UTI.
Application form contains detailed instructions for filling out the application as well as a list of documents to attach.

Please note that you will need to attach your photo and ID proof.
